Homunculus is a chilling psychological seinen manga by Hideo Yamamoto that follows Susumu Nakoshi, a 34-year-old homeless man living out of his car between a luxury hotel and a public park. Desperate for money, he agrees to undergo trepanation—the act of drilling a hole in the skull—as part of an experimental study led by eccentric medical student Manabu Itoh. The procedure awakens Nakoshi's "sixth sense," allowing him to see people's hidden traumas and suppressed insecurities as grotesque, surreal distortions known as "homunculi" whenever he closes his right eye. As he becomes entangled in the warped inner worlds of others, Nakoshi is forced to confront his own amnesia and the dark, pathological lies of his past.
