Set in 1930s England, Rule of Rose follows Jennifer, a young woman who is led to a dilapidated orphanage and subsequently trapped in a surreal, nightmare world aboard a massive airship. The narrative explores Jennifer's suppressed childhood traumas as she is forced to navigate the cruel social hierarchy of the "Red Crayon Aristocrats," a clique of young girls who subject her to psychological abuse and bizarre hazing rituals. Guided by a series of homemade storyboooks, Jennifer must uncover the truth behind a tragic airship accident and her relationship with a mysterious girl named Wendy, ultimately confronting the "imps" and monsters that manifest from her own fractured memories.

The gameplay is a blend of third-person survival horror and adventure-style puzzle solving, heavily centered on relying on your dog to help you. Players can command Brown to track specific scents, which is essential for finding key items and "gifts" required to progress through the game's monthly themed chapters. While the game features melee combat using improvised tools like pipes and kitchen knives, the mechanics are intentionally clunky to reflect Jennifer’s vulnerability, often making evasion a more viable strategy than direct confrontation. Survival depends on managing limited resources and health restoratives while navigating labyrinthine environments filled with hostile, doll-like creatures.
